No one is guaranteed a specific Honours Specialization. Assured-admission students get priority placement. Some modules fill entirely with them, and competitive-pool students are not considered at all for those. 75% is the floor to be eligible, not a number that gets you a module.

Getting into Med Sci is not getting into BMSc
Western states it directly: admission to Medical Sciences is not admission to the BMSc Program. You are a Medical Sciences student in Years 1 and 2 and apply to BMSc entering Year 3. Your Grade 12 average gets you to London. Your first two years of university decide whether you actually do BMSc, and in what.

Western makes no automatic alternative offers
If you want to be considered for both Medical Sciences and Health Sciences, list each as a separate choice on OUAC. Same for Kinesiology (EP).

A late offer is not a worse offer
Western reviews your application in every round from November to May, using whatever combination of Grade 11 and Grade 12 marks it has. Western states there is no disadvantage to being admitted in the final round. Grade 11 marks matter for early rounds.
